Fewer nighttime trips to the bathroom

โ€œWaking up three times a night was wrecking my sleep. A beta-sitosterol and saw palmetto formula cut those trips down to once.โ€

I'm 58 and started getting up two and three times a night to use the bathroom about a year ago. Annoying, but the bigger thing was the urgency during the day โ€” that "have to find a bathroom now" feeling that kicks in suddenly and doesn't tolerate negotiation. My PSA was normal, my urologist called it BPH (benign prostatic hyperplasia), age-typical, "lots of options."

He offered alpha blockers (works fast, side effects), 5-alpha-reductase inhibitors (works slowly, sexual side effects more concerning), and "you could try saw palmetto." I went home and read up on saw palmetto and it was a mess โ€” some studies positive, some negative, dosing all over the map, quality of the extract varying wildly.

RankOfSupplements' prostate category was where the picture cleared up. The writeup walked through why the negative meta-analyses generally used unstandardized extracts at sub-therapeutic doses, while the positive trials used the standardized lipidic extract at 320 mg/day. They also pointed out that the better studies in the modern era have moved to combination formulas (saw palmetto + pygeum + nettle root + zinc) which seem to outperform any single ingredient. Prostan Plus was their top-rated pick and contained all four at the doses used in the European clinical trials.

I gave it a 10-week trial with the urologist's blessing. Week one to four: nothing obvious. Week five I noticed I'd gotten through a Saturday morning of yard work without needing to come inside. Week seven I was sleeping through to 5am โ€” one bathroom trip down from three. Week ten I had a few full nights with no waking, which I hadn't had in over a year.

The day-time urgency improved less dramatically. I'd say it dropped from "very intrusive" to "manageable but still there." For the night-time symptoms, the improvement was real and I noticed it within a few months.

Honest framing: BPH doesn't go away โ€” it's a structural thing related to the prostate itself, and the supplement isn't shrinking the prostate the way the prescription medication would. What it's doing is reducing the inflammatory and DHT-related component on top, which buys some functional improvement without the side effects. If my symptoms were severe I'd take the prescription. For the moderate level I was at, the supplement was the right starting point.

Side effects: zero. Some men report mild GI upset on saw palmetto โ€” I take it with food and haven't had any issue. I also haven't noticed any sexual side effects, which is the thing I was worried about with the prescription option.

I've been on it for about eight months now. I retest my PSA annually because saw palmetto can mask elevated PSA in some men, which my urologist warned me about โ€” that's the one cautionary note for anyone starting it. Otherwise it's been a quiet, steady win.
